Job summary

Victorian Post Acute in San Francisco, CA is seeking a PRN Occupational Therapist (OTR) to join our in-house therapy team. You will evaluate residents, develop plans of care, and provide skilled OT services per established protocols.

New grads are encouraged to apply. This position offers flexible scheduling and the opportunity to work collaboratively with PTs, OTs, COTAs, SLPs, and nursing staff in a rehab-focused SNF.

Responsibilities

• Evaluate residents and develop individualized OT treatment plans in collaboration with the referring physician and interdisciplinary team.
• Provide skilled occupational therapy services according to the established plan of care.
• Supervise COTAs in accordance with California practice guidelines.
• Train and support Restorative Nursing Assistants as appropriate.
• Participate in interdisciplinary care conferences, rehab meetings, and discharge planning.
• Maintain accurate documentation including evaluations, notes, progress reports, and discharge summaries.
• Educate residents and families on home exercise programs, adaptive equipment, and safety recommendations.
• Recommend and coordinate appropriate DME to maximize resident independence and safety.

Key Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans in collaboration with the referring physician and interdisciplinary team.
• Provide skilled occupational therapy services according to the established plan of care.
• Supervise Certified Occupational Therapy Assistants (COTAs) in accordance with California practice guidelines.
• Train and support Restorative Nursing Assistants as appropriate.
• Participate in interdisciplinary care conferences, rehabilitation meetings, and discharge planning.
• Maintain accurate and timely documentation, including evaluations, daily treatment notes, progress reports, recertifications, and discharge summaries.
• Educate residents, families, and caregivers on home exercise programs, adaptive equipment, and safety recommendations.
• Recommend and coordinate appropriate durable medical equipment (DME) to maximize resident independence and safety.
• Report equipment concerns promptly to maintain a safe therapy environment.

Qualifications

• Current California Occupational Therapist (OTR) license in good standing.
• Strong communication and collaboration skills.
