Rossmoor Post Acute in Walnut Creek, CA is hiring a passionate Occupational Therapist (OTR) to join our in-house rehabilitation team. This full-time role focuses on delivering goal-oriented therapy to residents in a supportive, collaborative environment where clinical excellence and resident outcomes are prioritized.

You’ll develop individualized treatment plans, support ADLs and mobility, supervise COTAs, and coordinate care with nursing and the broader interdisciplinary team.

Qualifications

• Current California Occupational Therapist (OT) license in good standing.
• New graduates are encouraged to apply.

Responsibilities

• Develop and implement individualized occupational therapy treatment plans.
• Help residents improve independence with Activities of Daily Living (ADLs) and functional mobility.
• Evaluate resident progress and modify treatment plans as clinically appropriate.
• Supervise and support Certified Occupational Therapy Assistants (COTAs).
• Collaborate with nursing and interdisciplinary team members to coordinate resident care.
• Participate in Patient Care Conferences and Utilization Review meetings.
• Maintain accurate, timely, and compliant clinical documentation.
• Provide education and training to residents, caregivers, and families.
• Assist with safe and effective discharge planning.
• Communicate with physicians regarding treatment updates, resident progress, and care recommendations.

What You'll Do
