Rossmoor Post Acute in Walnut Creek, CA is seeking a motivated and compassionate Occupational Therapist (OTR) to join our in-house rehabilitation team. You will evaluate residents, develop individualized treatment plans, and provide skilled OT services to improve ADLs and independence while working closely with an interdisciplinary team.

Responsabilités

• Evaluate residents and develop individualized occupational therapy treatment plans.
• Provide skilled therapy to improve activities of daily living (ADLs) and independence.
• Monitor resident progress and adjust treatment plans as clinically indicated.
• Supervise and support Certified Occupational Therapy Assistants (COTAs).
• Collaborate with nursing and interdisciplinary team members to coordinate resident care.
• Participate in patient care conferences and utilization review meetings as needed.
• Maintain accurate, timely, and compliant clinical documentation.
• Educate residents, families, and caregivers on treatment plans and strategies for continued progress.
• Support safe and effective discharge planning.
• Communicate with physicians regarding resident progress, treatment updates, and care recommendations.
