Rossmoor Post Acute in Walnut Creek, CA is hiring an Occupational Therapist (OTR) to join our premier in-house rehabilitation team. This full-time role offers a starting rate of $60+ per hour DOE and provides a stable, facility-managed program designed for growth and mentorship.

Qualifications

• Current California Occupational Therapist (OT) license in good standing.
• New graduates welcome.

Responsibilities

• Develop and implement individualized occupational therapy treatment plans.
• Help residents improve independence with Activities of Daily Living (ADLs).
• Supervise and support Certified Occupational Therapy Assistants (COTAs).
• Collaborate with nursing and interdisciplinary teams to optimize resident care.
• Participate in Patient Care Conferences and Utilization Review meetings.
• Maintain accurate and timely documentation in compliance with state regulations.
• Provide caregiver and family education for safe discharge planning.
• Communicate with physicians regarding treatment updates and care recommendations.
