Job summary

White Blossom Care Center in San Jose, CA is seeking a licensed Occupational Therapist (OTR) to join our in-house rehabilitation program in a high-volume, resource-rich clinical environment.

We offer an expansive rehab gym, on-site modalities, career growth, and flexible scheduling options (FT/PT/PRN) with competitive hourly compensation of $50–$65 per hour DOE.

Qualifications

• Valid California Occupational Therapist (OT) license in good standing.
• Post-acute and long-term care settings experience desired.
• Commitment to teamwork, professional growth, and patient-centered outcomes.

Responsibilities

• Evaluation & Treatment: Perform comprehensive evaluations and deliver evidence-based occupational therapy interventions per physician orders
• Clinical Leadership: Supervise and support Certified Occupational Therapy Assistants (COTAs) in compliance with California State Practice Acts
• Interdisciplinary Collaboration: Participate in care conferences, utilization reviews, and rehab meetings to ensure coordinated, patient-centered care
• Documentation Excellence: Complete timely and accurate evaluations, daily notes, progress notes, and recertifications in accordance with OT Board standards
• DME & Patient Independence: Recommend, order, and train patients on durable medical equipment (DME) to promote safety and functional independence
• Education & Discharge Planning: Provide caregiver training and education to families and nursing staff to support positive discharge outcomes
