Job summary

White Blossom Care Center in San Jose, CA is seeking a dedicated Occupational Therapist (OTR) to join our in-house rehabilitation team. You will conduct comprehensive evaluations, develop individualized plans, and support a high-volume clinical environment.

Qualifications

• Current and valid California Occupational Therapist (OT) license.
• Experience in post-acute, skilled nursing, or long-term care is preferred.

Responsibilities

• Conduct comprehensive occupational therapy evaluations and develop individualized treatment plans based on physician orders and patient needs.
• Provide evidence-based occupational therapy interventions focused on improving functional mobility, independence, safety, and quality of life.
• Supervise and support COTAs in accordance with California regulations and applicable standards of practice.
• Participate in interdisciplinary care conferences, utilization reviews, and rehabilitation meetings.
• Complete timely and accurate evaluations, daily documentation, progress notes, and recertifications.
• Assess patients for appropriate durable medical equipment (DME) and provide training to promote safety and independence.
• Educate patients, families, caregivers, and nursing staff on therapeutic strategies and carryover programs.
• Participate in discharge planning and caregiver training to support successful transitions of care.

White Blossom Care Center is seeking a dedicated and compassionate Occupational Therapist (OTR) to join our established in-house rehabilitation team. This is an opportunity to work in a high-volume, resource-rich clinical environment where therapists have the space, resources, and support to provide exceptional patient care.

Our facility is home to one of the largest rehabilitation gyms in the Bay Area, offering an impressive environment for functional recovery, clinical variety, and professional growth.

Position Details

• Position: Occupational Therapist (OTR)
• Location: White Blossom Care Center - San Jose, CA
• Pay Rate: $50-$65 per hour, DOE
• Schedule: Full-Time, Part-Time, or PRN
• Program: Stable, In-House Therapy Team

Key Responsibilities
