Job summary

Concierge Home Care is seeking a licensed Occupational Therapist to provide skilled home health services in the Sarasota area. You will evaluate patients at home, design plans of care, and help clients regain independence and daily living skills.

The role emphasizes autonomy, flexible scheduling (including full-time, modified schedules, PRN), and use of AI-assisted charting and WellSky/Kinnser EMR to streamline documentation. A supportive leadership team and ongoing clinical training are offered.

Qualifications

• Active, unrestricted Florida OT license.
• CPR certification.
• Ability to supervise Certified Occupational Therapy Assistants (COTAs) as appropriate.
• Home health experience preferred but not required.

Responsibilities

• Conduct patient evaluations in patients' homes and assess the home environment.
• Develop individualized treatment plans based on physician orders.
• Provide therapy to improve functional mobility and daily living skills.
• Educate patients and caregivers on adaptive strategies and safety techniques.
• Collaborate with physicians and interdisciplinary care teams.

Your Role as a Home Health Occupational Therapist

As a Home Health Occupational Therapist , you will provide skilled therapy services in patients’ homes to help them regain independence, improve daily function, and safely perform activities of daily living.

Responsibilities include:

• Conduct patient evaluations and home environment assessments
• Develop individualized treatment plans based on physician orders
• Provide therapy focused on improving functional mobility and daily living skills
• Educate patients and caregivers on adaptive strategies and safety techniques
• Collaborate with physicians and interdisciplinary care teams
• Supervise and delegate care to Certified Occupational Therapy Assistants (COTAs) when appropriate
• Document visits efficiently using AI-assisted tools
