Job summary

Concierge Home Care in Spring Hill, FL is seeking an Occupational Therapist to deliver skilled home health therapy in patients’ homes, helping them regain independence and daily living skills.

You will evaluate, plan, and implement treatment, educate families, collaborate with physicians, and document visits with AI-assisted tools; supervision of COTAs where appropriate is possible. The role offers flexibility and autonomy with supportive leadership.

Qualifications

• Active, unencumbered Florida Occupational Therapist license is required.
• Ability to work in patient homes with autonomy and strong time management.
• Experience with home health settings and using EMR tools preferred.

Responsibilities

• Develop individualized therapy plans based on physician orders.

Your Role as a Home Health Occupational Therapist

As a Home Health Occupational Therapist , you will provide skilled therapy services in patients’ homes to help them regain independence, improve daily function, and safely perform activities of daily living.

Responsibilities include:

• Conduct patient evaluations and home environment assessments
• Develop individualized treatment plans based on physician orders
• Provide therapy focused on improving functional mobility and daily living skills
• Educate patients and caregivers on adaptive strategies and safety techniques
• Collaborate with physicians and interdisciplinary care teams
• Supervise and delegate care to Certified Occupational Therapy Assistants (COTAs) when appropriate
• Document visits efficiently using AI-assisted tools

Qualifications

• Active, unencumbered Florida Occupational Therapist license
• Valid driver’s license, reliable transportation, and auto insurance
• CPR certification
• Ability to supervise Certified Occupational Therapy Assistants (COTAs)
