Job summary

Concierge Home Care in Venice, FL is seeking a dedicated Occupational Therapist for home health. You will deliver skilled therapy in patients' homes, helping them regain independence and perform daily living activities.

You'll plan evaluations, create individualized treatment plans, educate patients and caregivers, and collaborate with physicians. We offer flexible scheduling (FT/PRN/1099), AI-assisted charting, mentorship, and growth opportunities to support your work-life balance.

Qualifications

• Active Florida Occupational Therapist license is required.
• Driver's license and reliable transportation.
• CPR certification is required.

Responsibilities

• Evaluate patients and perform home environment assessments.

• Improve functional mobility and daily living skills.
• Educate patients and caregivers on adaptive strategies and safety techniques.

Your Role as a Home Health Occupational Therapist

As a Home Health Occupational Therapist , you will provide skilled therapy services in patients' homes to help them regain independence, improve daily function, and safely perform activities of daily living.

Responsibilities include:

• Conduct patient evaluations and home environment assessments
• Develop individualized treatment plans based on physician orders
• Provide therapy focused on improving functional mobility and daily living skills
• Educate patients and caregivers on adaptive strategies and safety techniques
• Collaborate with physicians and interdisciplinary care teams
• Supervise and delegate care to Certified Occupational Therapy Assistants (COTAs) when appropriate
• Document visits efficiently using AI-assisted tools

• Active, unencumbered Florida Occupational Therapist license
• Valid driver's license, reliable transportation, and auto insurance
• CPR certification
• Ability to supervise Certified Occupational Therapy Assistants (COTAs)
