Job summary

Concierge Home Care in Sarasota, FL is seeking a licensed Occupational Therapist to provide skilled home health services in patients’ homes. You will assess home environments, develop personalized treatment plans, and improve independence in activities of daily living.

The role offers autonomy, flexible scheduling (FT, modified schedules, PRN, contractor-1099), AI-assisted documentation, and a supportive leadership team with opportunities for growth.

Qualifications

• Active Florida Occupational Therapist license is required.
• Valid driver’s license, reliable transportation, and auto insurance.
• CPR certification is required.
• Ability to supervise Certified Occupational Therapy Assistants (COTAs).
• Home health experience preferred but not required.

Responsibilities

• Conduct patient evaluations and home environment assessments.
• Develop individualized treatment plans based on physician orders.
• Provide therapy focused on improving functional mobility and daily living skills.
• Educate patients and caregivers on adaptive strategies and safety techniques.
• Collaborate with physicians and interdisciplinary care teams.
• Supervise and delegate care to COTAs when appropriate.
• Document visits efficiently using AI-assisted tools.

Your Role as a Home Health Occupational Therapist

As aHome Health Occupational Therapist, you will provide skilled therapy services in patients’ homes to help them regain independence, improve daily function, and safely perform activities of daily living.
