Job summary

Life Care Centers of America is seeking an Occupational Therapist to provide direct rehab care in SNF settings. You will work with physicians to implement OT plans, collaborate with an interdisciplinary team, and help patients achieve functional goals in a patient-centered environment.

New grads welcome; NBCOT certification required on hire and active state license maintained. CPR certification is required and ongoing. Proficiency with MS Word/Excel and email is expected.

Qualifications

• NBCOT certification required on hire; renewal optional
• Graduate of an accredited OT program (BSOT or MSOT)
• Must maintain active state license in good standing
• CPR certification required on hire or during orientation; current throughout employment
• New grads welcome to apply; 1 year experience preferred

Responsibilities

• Follow occupational therapy treatment plans and goals
• Collaborate with interdisciplinary team to plan patient care
• Chart timely and accurately in therapy records
• Liaise with patients and families to plan for needs
• Maintain patient rights, safety and confidentiality
