Job summary

Human Touch Home Health is seeking a compassionate Occupational Therapist (OT) for PRN home health opportunities throughout Washington, DC. You will perform in-home evaluations, develop individualized treatment plans, and help patients regain independence in activities of daily living.

Qualifications

• Active Washington, DC Occupational Therapist License Required.
• Graduate of an accredited Occupational Therapy program.
• Strong clinical assessment and communication skills.
• Dependable transportation to patient visits.

Responsibilities

• Complete occupational therapy evaluations in patients' homes.
• Develop individualized treatment plans.
• Help patients improve independence with activities of daily living.
• Provide therapeutic interventions to improve strength, coordination, mobility, and function.
• Assess home safety and recommend modifications or adaptive equipment.
• Educate patients and caregivers on safety and daily living techniques.
• Monitor patient progress and adjust treatment plans as needed.
• Coordinate care with physicians, nurses, therapists, and other team members.
• Complete progress assessments and discharge planning.
• Complete accurate and timely documentation using WellSky.

What You'll Do
