Job summary

Human Touch Home Health is actively seeking a compassionate Occupational Therapist (OT) to join our team serving Southern Prince George's County, MD, including Clinton, Fort Washington, Oxon Hill, Temple Hills and surrounding areas. This PRN role offers flexible scheduling, independence, and the chance to make meaningful daily impact while delivering in-home therapy.

You will evaluate, treat, and document patient progress, building personalized plans and collaborating with physicians and nurses.

Qualifications

• Active Maryland Occupational Therapist License Required.
• Valid Driver's License and Reliable Transportation
• Strong Clinical Assessment, Communication, and Documentation Skills
• Home Health Experience Preferred but Not Required
• OASIS Experience Preferred
• Experience with WellSky Home Health is a plus
• Passion for Delivering Exceptional Patient‑Centred Care

Responsibilities

• Complete occupational therapy evaluations, reassessments, and discharge visits
• Develop individualized treatment plans based on patient goals and functional needs
• Provide skilled occupational therapy services in patients' homes
• Assess home environments and recommend adaptive equipment and safety modifications
• Educate patients and caregivers on home safety, adaptive equipment, energy conservation techniques, and activities of daily living
• Monitor patient progress and modify treatment plans as appropriate
• Collaborate with physicians, nurses, and the interdisciplinary care team
• Complete timely and accurate clinical documentation using WellSky
