Job summary

Human Touch Home Health is seeking a Physical Therapist (PT) for PRN home health opportunities throughout Washington, DC. Complete physical therapy evaluations in patients' homes and develop individualized plans of care.

Provide therapeutic exercise, gait, balance, transfer and mobility training, while assessing home safety and fall risks. Documentation via WellSky and coordination with physicians, nurses, and the care team are required.

Qualifications

• Active Washington, DC Physical Therapist License required.
• Graduate of an accredited Physical Therapy program.
• Strong clinical assessment and communication skills.

Responsibilities

• Perform home-based PT evaluations and develop plans of care.
• Provide therapeutic exercise, gait, balance, transfer and mobility training.
• Assess home safety and fall risk; educate patients and caregivers.
• Monitor progress and adjust treatment plans; complete progress notes and discharge planning.
• Coordinate care with physicians, nurses, PTAs and the care team.

What You'll Do

• Complete physical therapy evaluations in patients' homes
• Develop individualized plans of care
• Provide therapeutic exercise, gait, balance, transfer, and mobility training


• Assess home safety and fall risks
• Educate patients and caregivers
• Monitor progress and adjust treatment plans


• Complete progress assessments and discharge planning
• Coordinate care with physicians, nurses, PTAs, and other team members


• Complete accurate and timely documentation using WellSky

Qualifications

• Active Washington, DC Physical Therapist License Required
• Graduate of an accredited Physical Therapy program


• Strong clinical assessment and communication skills


• Dependable transportation to patient visits


• Ability to work independently in patients' homes


• Home Health and OASIS experience preferred, but not required


• WellSky experience is a plus
