Job summary

Human Touch Home Health is seeking an Occupational Therapist (OT) for PRN home health opportunities throughout Washington, DC. This role focuses on one-on-one patient care in patients' homes, helping individuals regain independence with daily activities.

The OT will develop personalized treatment plans, perform evaluations, and monitor progress while coordinating with the care team. Flexible scheduling supports caseload control and professional growth.

Qualifications

• Active Washington, DC Occupational Therapist License is required.
• Graduate of an accredited OT program is required.
• Strong clinical assessment and communication skills.

What You'll Do

• Complete occupational therapy evaluations in patients' homes
• Develop individualized treatment plans
• Help patients improve independence with activities of daily living
• Provide therapeutic interventions to improve strength, coordination, mobility, and function
• Assess home safety and recommend modifications or adaptive equipment
• Educate patients and caregivers on safety and daily living techniques
• Monitor patient progress and adjust treatment plans as needed
• Coordinate care with physicians, nurses, therapists, and other team members
• Complete progress assessments and discharge planning
• Complete accurate and timely documentation using WellSky

Qualifications

• Active Washington, DC Occupational Therapist License Required
• Graduate of an accredited Occupational Therapy program
• Strong clinical assessment and communication skills
• Dependable transportation to patient visits
• Ability to work independently in patients' homes
• Home Health and OASIS Experience Preferred, but Not Required
• WellSky Experience is a Plus
