Job summary

Human Touch Home Health is seeking a compassionate and skilled Occupational Therapist (OT) to join our team serving patients throughout Richmond, Henrico, Chesterfield, Mechanicsville, Glen Allen, and surrounding communities.

This PRN opportunity offers flexibility and independence, mileage reimbursement, and the chance to build a caseload that fits your availability, while delivering one-on-one home health care and collaborating with a supportive clinical leadership.

Qualifications

• Active Virginia Occupational Therapist License required.
• Valid Driver's License and reliable transportation.
• Strong clinical assessment and documentation skills.
• Excellent communication and interpersonal skills.
• Home Health experience preferred but not required.
• Experience with OASIS is a plus.

Responsibilities

• Complete occupational therapy evaluations, reassessments, and discharge visits.
• Develop individualized treatment plans based on patient goals and functional needs.
• Provide skilled occupational therapy services in patients' homes.
• Assess home environments and recommend adaptive equipment and safety modifications.
• Educate patients and caregivers on therapeutic techniques, home safety, and activities of daily living.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians and interdisciplinary team members.
• Complete timely and accurate clinical documentation.
