Job summary

Human Touch Home Health in Richmond, VA is seeking a compassionate Occupational Therapist (OT) to join our PRN team serving Richmond and surrounding communities. You’ll evaluate and treat patients in their homes, build a personalized care plan, and support independence.

This PRN role offers flexible scheduling, professional autonomy, and the chance to enhance patients'' quality of life through meaningful daily activities. Virginia OT license and reliable transport are required.

Qualifications

• Licensed Virginia Occupational Therapist active and in good standing.
• Valid driver''s license with reliable transportation.
• Strong assessment, documentation, and communication skills.
• Experience in home health or OASIS is advantageous.

Responsibilities

• Complete occupational therapy evaluations, reassessments, and discharge visits.
• Develop individualized treatment plans based on patient goals.
• Provide skilled occupational therapy services in patients'' homes.
• Assess home environments and recommend safety modifications.
• Educate patients and caregivers on techniques and daily living activities.
• Monitor patient progress and modify plans as needed.
• Collaborate with physicians and interdisciplinary team members.
• Complete timely and accurate clinical documentation.

• Collaborate with physicians and interdisciplinary team members
• Complete timely and accurate clinical documentation

Qualifications

• Active Virginia Occupational Therapist License Required
• Valid Driver's License and Reliable Transportation
• Strong Clinical Assessment and Documentation Skills
• Excellent Communication and Interpersonal Skills
• Home Health Experience Preferred but Not Required
• Experience with OASIS is a plus
