Human Touch Home Health is seeking a compassionate and skilled Occupational Therapist (OT) to join our team serving patients across Northern Virginia. This PRN opportunity offers flexible scheduling, autonomy to create individualized treatment plans, and one-on-one patient care in patients' homes.

Qualifications

• Active Virginia OT license required.
• Strong clinical assessment, communication, and documentation skills.
• Home health experience preferred but not required.
• OASIS experience helpful.
• Experience with WellSky Home Health is a plus.

Responsibilities

• Complete OT evaluations, reassessments, and discharge visits.
• Develop individualized treatment plans based on patient goals and needs.
• Provide skilled OT services in patients' homes.
• Assess home environments and recommend adaptive equipment and safety modifications.
• Educate patients and caregivers on techniques, energy conservation, and daily living activities.
• Collaborate with physicians and the care team; document using WellSky.

As a Home Health Occupational Therapist, you'll help patients regain the skills, strength, and confidence they need to perform everyday activities safely while remaining in the comfort of their own homes.

What You\'ll Do

• Complete occupational therapy evaluations, reassessments, and discharge visits
• Develop individualized treatment plans based on patient goals and functional needs
• Provide skilled occupational therapy services in patients\' homes
• Assess home environments and recommend adaptive equipment and safety modifications
• Educate patients and caregivers on therapeutic techniques, energy conservation, home safety, adaptive equipment, and activities of daily living
• Monitor patient progress and modify treatment plans as appropriate
• Collaborate with physicians and the interdisciplinary care team
• Complete timely and accurate clinical documentation using WellSky

Qualifications

• Active Virginia Occupational Therapist License Required
• Valid Driver\'s License and Reliable Transportation
• Strong Clinical Assessment, Communication, and Documentation Skills
• Home Health Experience Preferred but Not Required
• OASIS Experience Preferred
• Experience with WellSky Home Health is a plus
