Job summary

Human Touch Home Health in Virginia is seeking a compassionate Occupational Therapist to join our team delivering in-home care across Northern Virginia communities. This PRN role offers flexible scheduling and the autonomy to build a caseload in patients' homes.

You will perform evaluations, develop individualized treatment plans, provide skilled OT services, and educate patients and caregivers, while collaborating with physicians and the care team to ensure safe, meaningful outcomes.

Qualifications

• Active Virginia Occupational Therapist License Required.
• Valid Driver's License and Reliable Transportation.
• Home Health Experience Preferred but Not Required.
• OASIS Experience Preferred.
• Experience with WellSky Home Health is a plus.
• Passion for Delivering Exceptional Patient-Centered Care.

Responsibilities

• Complete occupational therapy evaluations, reassessments, and discharge visits.
• Develop individualized treatment plans based on patient goals and functional needs.
• Provide skilled occupational therapy services in patients' homes.
• Assess home environments and recommend adaptive equipment and safety modifications.
• Educate patients and caregivers on therapeutic techniques, energy conservation, home safety, adaptive equipment, and activities of daily living.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians and the interdisciplinary care team.
• Complete timely and accurate clinical documentation using WellSky.
