Job summary

Tice Valley Post Acute in Walnut Creek, California is seeking a compassionate Occupational Therapist (OT) to join our rehabilitation team on a full-time basis. This role offers competitive pay and the opportunity to help residents regain independence in daily activities.

You will evaluate residents, develop individualized treatment plans, and collaborate with an interdisciplinary team to promote safety, dignity, and quality of life.

Qualifications

  • Current and valid California OT license.
  • Graduate of an accredited OT program.
  • Experience in skilled nursing, rehabilitation, or long-term care preferred.
  • Strong clinical assessment, treatment planning, and documentation skills.
  • Excellent communication and interpersonal skills.

Responsibilities

  • Evaluate residents' functional abilities and develop individualized treatment plans.
  • Provide occupational therapy services to improve residents' strength, mobility, coordination, and independence with daily activities.
  • Implement therapeutic interventions based on residents' individual needs and care plans.
  • Assist residents with activities of daily living (ADLs) including dressing, grooming, bathing, feeding, and other functional tasks.
  • Assess and address safety, mobility, and environmental needs.
  • Monitor resident progress and modify treatment plans as appropriate.
  • Document evaluations, treatments, progress, and outcomes accurately.
  • Collaborate with physicians, nurses, CNAs, physical therapists, speech therapists, and other members of the interdisciplinary team.
  • Educate residents and caregivers on therapeutic techniques, exercises, and strategies to support independence.
  • Recommend adaptive equipment and assistive devices when appropriate.
  • Participate in care planning and interdisciplinary meetings.
  • Maintain compliance with facility policies, professional standards, and applicable regulations.
  • Promote resident dignity, safety, independence, and overall quality of life.
  • Perform other occupational therapy-related duties as assigned.

Position Details
  • Position: Occupational Therapist (OT)
  • Employment Type: Full-Time
  • Location: Tice Valley Post Acute
  • Pay Rate: $55.00-$60.00 per hour, DOE
