Job summary

Santa Rosa Post Acute is seeking a compassionate Occupational Therapist to join our rehabilitation team in Santa Rosa, CA. You will assess residents, develop individualized treatment plans, and deliver skilled OT interventions to improve ADLs and independence.

Qualifications

• California OT license in good standing
• Graduate of an accredited OT program
• Experience in geriatric or post-acute rehab preferred
• Familiarity with EMR systems (Casamba/Net Health/PointClickCare) a plus

Responsibilities

• Evaluate residents and develop individualized occupational therapy treatment plans
• Provide skilled occupational therapy interventions in accordance with physician orders and established care plans
• Help patients improve performance in activities of daily living (ADLs), self-care tasks, mobility, cognition, and functional independence
• Collaborate with Physical Therapists, Speech Therapists, nursing staff, physicians, and families to coordinate care
• Complete timely and accurate documentation in compliance with regulatory and facility requirements
• Participate in interdisciplinary team meetings, care conferences, and discharge planning
• Educate patients, families, and caregivers regarding therapeutic techniques and home programs
• Maintain compliance with all federal, state, and professional standards of practice

• Skilled nursing, post-acute, rehabilitation, or geriatric experience preferred
• New graduates with strong clinical rotations are encouraged to apply
• Experience with Casamba, Net Health, PointClickCare, or similar EMR systems is a plus
• Strong communication, organizational, and teamwork skills
