Job summary

McClure Post Acute is seeking a compassionate Occupational Therapist (OT) to join our in-house rehabilitation team in Oakland, CA. We offer full-time, part-time, and PRN options with a supportive, family-oriented culture and mentorship for new graduates.

Responsibilities include evaluating residents, creating and adjusting treatment plans, and collaborating with physicians and nursing staff to maximize independence and safety in daily activities.

Qualifications

  • Current and active California OT license in good standing.
  • Strong collaboration within interdisciplinary teams.
  • New graduates are encouraged to apply with structured mentorship.

Responsibilities

  • Perform comprehensive occupational therapy evaluations and develop individualized treatment plans.
  • Provide skilled interventions focusing on ADLs, functional independence, safety, and quality of life.
  • Monitor progress and modify plans to achieve optimal outcomes.
  • Collaborate with physicians, nursing staff, therapists, and other team members.
  • Educate residents and families on adaptive equipment, home safety, and strategies for independence.
  • Participate in care planning and discharge planning as appropriate.
  • Maintain accurate, timely, and compliant clinical documentation.
