Lone Tree Post Acute in Antioch, CA is seeking an Occupational Therapist to join our in-house rehabilitation team. You will evaluate residents, develop individualized, evidence-based treatment plans, and deliver OT services to promote functional independence.

Collaborate with physicians, nursing, and the interdisciplinary care team to achieve optimal outcomes. You will supervise COTAs and students, participate in care conferences and discharge planning, and educate residents and families on home

Qualifikationen

• Current, active California Occupational Therapist (OT) license.
