Lone Tree Post Acute in Antioch, CA is seeking an Occupational Therapist (OT) to join our in-house rehabilitation team. You will evaluate residents and develop individualized, evidence-based treatment plans to promote functional independence and quality of life.

Deliver skilled OT services, collaborate with physicians and nursing staff, supervise COTAs and therapy students, participate in care conferences and discharge planning, and educate residents, families on home safety.

Qualifications

• Current, active California Occupational Therapist (OT) license.
