Job summary

Human Touch Home Health is seeking a compassionate Occupational Therapist (OT) to join our team serving communities in Northern Northampton County, PA, including Pen Argyl, Wind Gap, Bangor, Portland, Mount Bethel, Roseto, East Bangor, and surrounding areas.

As a PRN Home Health OT, you will evaluate patients in their homes, develop individualized treatment plans, educate caregivers, and collaborate with physicians and the care team.

Qualifications

• Active Pennsylvania OT license required.
• Valid driver's license and reliable transportation.
• Experience in home health preferred but not required.
• OASIS experience preferred.
• Experience with WellSky Home Health is a plus.
• Passion for delivering patient-centered care.

Responsibilities

• Complete occupational therapy evaluations, reassessments, and discharge visits.
• Develop individualized treatment plans based on patient goals and functional needs.
• Provide skilled occupational therapy services in patients' homes.
• Assess home environments and recommend adaptive equipment and safety modifications.
• Educate patients and caregivers on home safety and activities of daily living.
• Monitor patient progress and modify treatment plans as appropriate.
• Collaborate with physicians, nurses, and the interdisciplinary care team.
• Complete timely and accurate clinical documentation using WellSky.

What You'll Do

Qualifications

• Active Pennsylvania Occupational Therapist License Required
• Valid Driver\'s License and Reliable Transportation
• Strong Clinical Assessment, Communication, and Documentation Skills
• Home Health Experience Preferred but Not Required
• OASIS Experience Preferred
• Experience with WellSky Home Health is a plus
• Passion for Delivering Exceptional Patient-Centered Care
