Human Touch Home Health is seeking a compassionate Occupational Therapist (OT) to join our team serving patients throughout Washington, DC. This PRN opportunity offers flexibility, professional independence, and the chance to make a meaningful daily impact.

Qualifications

• Active Washington, DC Occupational Therapist License is required.
• Valid Driver's License and reliable transportation.
• Strong assessment, communication, and documentation skills.
• Home health experience preferred but not required.

Responsibilities

• Perform occupational therapy evaluations, reassessments, and discharge visits.
• Develop individualized treatment plans based on patient goals and needs.
• Deliver skilled OT services in patients' homes.
• Assess home environments and recommend adaptive equipment and safety modifications.
• Educate patients and caregivers on therapeutic techniques and activities of daily living.
• Monitor progress and adjust plans as needed.
• Collaborate with physicians and the care team.
• Complete clinical documentation using WellSky.

What You'll Do

• Complete occupational therapy evaluations, reassessments, and discharge visits
• Develop individualized treatment plans based on patient goals and functional needs
• Provide skilled occupational therapy services in patients' homes
• Assess home environments and recommend adaptive equipment and safety modifications
• Educate patients and caregivers on therapeutic techniques, home safety, energy conservation, and activities of daily living
• Monitor patient progress and modify treatment plans as appropriate
• Collaborate with physicians and the interdisciplinary care team
• Complete timely and accurate clinical documentation using WellSky

Qualifications

• Active Washington, DC Occupational Therapist License Required
• Valid Driver's License and Reliable Transportation
• Strong Clinical Assessment, Communication, and Documentation Skills
• Home Health Experience Preferred but Not Required
• OASIS Experience is a Plus
• Passion for Delivering Exceptional Patient-Centered Care
