Job summary

Adelphi Nursing & Rehabilitation Center is seeking an Occupational Therapist (OT) for a full-time position. You will assess residents, develop and implement treatment plans, and work with a multidisciplinary team to deliver patient-centered rehabilitation.

Qualifications

• Must hold a valid OT license and state registration.
• Experience in long-term care and/or geriatric care preferred.
• Flexible start date and reliable attendance expected.

Responsibilities

• Provide direct patient care to assess medical condition, function, and rehabilitation potential.
• Establish and administer a treatment program with goals aligned to patient capacity and physician direction.
• Document patient status and educate patients and families in appropriate rehabilitation methods.
• Collaborate with all disciplines to deliver patient-centered care.

Duties and Responsibilities:

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.
• Document and record the patients’ condition and educate patients and families in an appropriate physical therapy method.
• Collaborates with all disciplines to provide best patient centered treatment and care.

Requirements/Qualifications:

• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred
