Job summary

Adelphi Nursing & Rehabilitation Center is seeking a dedicated Occupational Therapist (OT) to join our full-time team. You will provide direct patient care, assess functional abilities, and help develop individualized rehabilitation plans for residents in a compassionate, team-based setting.

Qualifications

• Must hold a valid OT license and registration for the state of practice.
• Experience in long-term care and/or geriatric settings preferred.
• Demonstrated ability to work as part of a multidisciplinary team.

Responsibilities

• Provide direct patient care to assess medical condition, functional abilities, and rehabilitation potential.
• Establish and implement treatment plans with goals aligned to patient capacity and physician direction.
• Document patient status and educate patients and families on therapy approaches.
• Collaborate with all disciplines to ensure patient-centered care.

Duties and Responsibilities

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.
• Document and record the patients’ condition and educate patients and families in an appropriate physical therapy method.
• Collaborates with all disciplines to provide best patient centered treatment and care.
