Job summary

Adelphi Nursing & Rehab Center is seeking a dedicated Occupational Therapist (OT) for a full-time role. You will provide direct patient care, assess medical conditions and functional abilities, and establish treatment plans under physician direction.

Qualifications

• Must have a valid professional license and registration for state of practice.

• Establish and administer a treatment program with goals determined by patient capacity under physician direction.
• Document and educate patients and families on the treatment plan and progress.
• Collaborates with all disciplines to provide patient-centered care.

Duties and Responsibilities:

• Providing direct patient care to assess their medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patient’s capacity and tolerance under the direction of the Physician.
• Document and record the patients’ condition and educate patients and families in an appropriate physical therapy method.
• Collaborates with all disciplines to provide best patient centered treatment and care.
