Job summary

Adelphi Nursing & Rehab Center is seeking a Physical Therapist to join our patient-centered care team. This full-time (also part-time considered) role involves direct patient care, rehabilitation planning, and collaboration with physicians and staff.

Qualified candidates hold a valid state license, with LTC/geriatric experience preferred. We offer competitive wages, PTO, and insurance, plus a $10K retention bonus for full-time staff.

• Must have a valid professional license and registration for state of practice.
• Experience with long-term care and/or geriatric care is preferred

Responsibilities

• Providing direct patient care to assess medical condition, functional capabilities, limitations and restrictions and potential for rehabilitation.
• Establish and administer a treatment program with specific goals determined according to the patient's capacity and tolerance under the direction of the Physician.
• Document and record the patients' condition and educate patients and families in an appropriate physical therapy method.
• Collaborates with all disciplines to provide best patient centered treatment and care.
