Job summary

Trilogy Home Healthcare, a Humana company, is seeking a dedicated Occupational Therapist to join our award-winning team for home health visits in Sumter County, Florida. This PRN/per diem role offers flexible scheduling and clinical autonomy to deliver personalized care.

You will collaborate with physicians and the clinical team to help patients regain mobility, balance, and independence after illness or surgery, while educating families and ensuring safe home environments.

Qualifications

• OT degree and Florida licensure required.
• Home health experience preferred; clinical autonomy in a PRN role.

Responsibilities

• Deliver compassionate, high-quality OT care in patients' homes.

What You Bring:

• Graduate of an accredited school of Occupational Therapy Program (AOTA-approved)
• Active OT license in Florida
• Valid Florida drivers license, active vehicle insurance, and clear MVR
• 1+ years of experience as an OT required, Home Health experience preferred
• Strong initiative and independent judgement
• Alignment with the agency's mission and values
