Continuum Therapy Partners is seeking a licensed Occupational Therapist to join our Melrose, MN team. The role focuses on evaluating patients, developing individualized treatment plans, and guiding families on home safety and equipment needs.

This is a full-time daytime position with benefits and growth opportunities. Qualified candidates hold a Master's or Doctorate in OT, an active state license, and NBCOT certification.

Qualifications

• Master's or Doctorate Degree in Occupational Therapy is required.
• NBCOT Certification is required and licensure must be current.
• 1 year of OT experience is preferred.

Responsibilities

• Evaluates and develops individualized treatment plans to improve patients' functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.

Occupational Therapist Requirements & Responsibilities:

• Master's or Doctorate Degree in Occupational Therapy required

• Evaluates and develops individualized treatment plans to improve patients' functional independence
