Continuum Therapy Partners, LLC invites an Occupational Therapist to join our clinician-led network across 275 locations. You will evaluate, plan, and deliver patient-centered interventions to improve functional independence in skilled care settings.

Requirements include a Master’s or Doctorate in Occupational Therapy, an active state license, NBCOT certification, and a preference for 1 year of OT experience. Full-time daytime schedule at Norwood Crossing, Chicago, IL with comprehensive benefits.

Qualifications

• Master’s or Doctorate degree in Occupational Therapy required.
• Current, unencumbered state Occupational Therapy License; NBCOT Certification required.
• 1 year of previous experience as an Occupational Therapist preferred.

Responsabilités

• Evaluates and develops individualized treatment plans to improve patients’ functional independence.
• Documents patient progress and modifies treatment plans based on clinical outcomes.
• Educates patients and families on adaptive equipment and home safety recommendations.
