Job summary

Evolve Therapy Services is seeking a Physical Therapist for an in-house Skilled Nursing Facility in North Platte, NE. This full-time role offers a $10,000 relocation or sign-on bonus and flexible scheduling in addition to salaried compensation.

Responsibilities include providing PT services, documentation, evaluations, and developing treatment plans while collaborating with an interdisciplinary team to ensure high-quality resident care.

Qualifications

• Bachelor of Science in Physical Therapy from an accredited program.
• Active Physical Therapy license in state(s) of practice.
• Ongoing continuing education to maintain licensure.
• Commitment to professional development and compliance.

Responsibilities

• Provide physical therapy services to residents per standards.
• Document and bill therapies per procedures.
• Evaluate and plan resident therapy programs.
• Screen residents regularly to determine intervention needs.
• Screen residents referred for therapy to assess needs.
• Evaluate residents to inform treatment planning.
• Develop treatment plans with goals and methods.
• Communicate findings and plans to residents and families.
• Collaborate with interdisciplinary team.
• Complete CMS-compliant documentation in NetHealth EMR.
• Discharge duties and ensure quality resident care.
• Perform other duties as assigned.
