Job summary

Innovate Rehab and Wellness in Center, MO is seeking a licensed Occupational Therapist to join our skilled nursing facility team. You will evaluate patients, develop individualized treatment plans, and help improve daily function and independence.

This full-time role offers a Monday–Friday schedule with a competitive hourly rate based on experience, and benefits starting day one. You will collaborate with a dedicated care team to achieve strong clinical outcomes.

Qualifications

• Master’s or Doctorate in Occupational Therapy (accredited program).
• Valid state OT license.
• Experience as an OT preferred; new grads welcome.
• Excellent communication and problem-solving skills.
• Passion for patient-centered care.

Responsibilities

• Evaluate patients’ functional abilities and therapy needs.
• Create and deliver individualized treatment plans.

Position Summary

The Occupational Therapist will:

• Evaluate patients’ functional abilities and therapy needs


• Create and deliver individualized treatment plans

• Support patients in regaining independence and quality of life

Key Responsibilities

• Conduct thorough OT evaluations


• Develop and adjust personalized treatment plans


• Use therapeutic techniques and adaptive equipment


• Train patients and caregivers on exercises and assistive devices


• Educate on strategies to improve function and safety


• Collaborate with PT, SLP, physicians, and the full care team


• Document patient progress and treatment changes accurately


• Stay current with OT best practices and research
