Job summary

Innovate Rehab and Wellness is seeking a licensed Occupational Therapist to join our team in a skilled nursing setting. This full-time role focuses on individual therapy plans, functional evaluation, and helping patients regain independence.

You will work Monday–Friday with a flexible schedule, collaborate with PT/SLP and physicians, and benefit from start-day-1 medical, dental, vision, and comprehensive support for professional growth.

Qualifications

• Master's or Doctorate in Occupational Therapy (accredited program).
• Valid state OT license.
• Experience as an OT preferred (new grads welcome).
• Strong communication and problem-solving skills.
• Passion for patient-centered care.
• CPR certification preferred.

Responsibilities

• Evaluate patients' functional abilities and therapy needs.
• Create and deliver individualized treatment plans.
• Help patients improve motor skills, coordination, and daily function.
• Support patients in regaining independence and quality of life.
• Conduct thorough OT evaluations.
• Develop and adjust personalized treatment plans.
• Train patients and caregivers on exercises and assistive devices.
• Document patient progress and treatment changes accurately.
