Job summary

Innovate Rehab and Wellness is seeking a Licensed Physical Therapist to join our skilled nursing facility team. You will evaluate patients, create individualized treatment plans, and lead hands-on therapy to restore movement and quality of life.

Work with an interdisciplinary team to ensure optimal recovery, stay current with best practices, and educate patients and families on home programs for injury prevention and wellness.

Qualifications

• Doctorate or Master’s degree in Physical Therapy (accredited program).
• Valid state PT license.
• Clinical experience preferred; new grads welcome.

Responsibilities

• Perform comprehensive PT evaluations.
• Develop personalized, evidence-based treatment plans.
• Provide hands-on therapy: manual techniques, therapeutic exercise, mobility training.
• Educate patients and caregivers on home programs and injury prevention.
• Track and document progress; adjust plans as needed.
• Collaborate with physicians, nurses, OTs, and other clinicians.
• Stay current with PT best practices through continuing education.
• Ensure compliance with regulatory and ethical standards.
