Job summary

Innovate Rehab and Wellness in Sedalia, MO is seeking a licensed Occupational Therapist for a full-time SNF role. You will evaluate patients, create individualized plans, and guide exercises to restore function and independence.

Collaborative care and evidence-based practice are emphasized to achieve quality outcomes. Applicants should hold a Master’s or Doctorate in OT, possess a valid state license, and be capable of coordinating with the care team.

Qualifications

• Master’s or Doctorate in Occupational Therapy from an accredited program.
• Valid state OT license required to practice.
• CPR certification preferred; experience in SNF is a plus.

Responsibilities

• Conduct OT evaluations and develop individualized treatment plans.
• Train patients and caregivers on exercises and assistive devices.
• Help patients improve motor skills, coordination and daily function.
• Document progress and adjust treatment plans as needed.
• Collaborate with PT, SLP, physicians and care team.
