Job summary

Eden Senior Care seeks a PRN Occupational Therapist (OT) to provide rehabilitation services at Edenbrook of Oshkosh, part of the Eden Senior Care Network. The OT will evaluate residents, implement treatment plans, and supervise COTAs to maximize ADL/IADL performance across skilled nursing settings.

The role emphasizes quality care, collaboration with families, and ongoing progress documentation in a fast-paced environment. PRN hours available with sign-on bonus.

Qualifications

• Valid license to practice occupational therapy.
• BS degree or greater in occupational therapy.
• Experience in skilled nursing, home health, or rehabilitation settings preferred.

Responsibilities

• To provide quality Occupational therapy services and initiates referrals when appropriate, in accordance with established standards of practice, therapy department procedures, and productivity standards.
• To evaluate residents to obtain data necessary for treatment planning and implementation and conduct specialized evaluations as indicated.
• To supervise and direct Certified Occupational Therapy Assistants (COTAs) in the implementation of the treatment plan.
• Apply interventions to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL)
• Recommend adaptive equipment and train in its use
• Guide and educate family members and caregivers
• Observe, note, and report on progress of long- and short-term treatment goals
