Job summary

Diamond Care Group in Florida is seeking an experienced Occupational Therapist to provide comprehensive home health services, focusing on improving patient independence and safety within their home environment.

You will conduct in-home evaluations, develop personalized treatment plans, and assist patients with ADLs, adapting equipment and home safety strategies to each home setting.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Active Florida OT license.
• Home health experience preferred.
• OASIS experience strongly preferred.
• Strong documentation and time management skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct in-home evaluations and functional assessments.

• Assist patients with ADLs (bathing, dressing, grooming, toileting).
• Train patients on adaptive equipment and home safety modifications.
• Complete OASIS documentation as required.
