Job summary

Palms-Home-Care is expanding its team and seeks an experienced Occupational Therapist to deliver home health services focused on improving independence and safety in the patient’s home.

You will conduct in-home evaluations, develop tailored treatment plans, and support ADLs while coordinating with nursing and therapy colleagues to optimize outcomes.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Active Florida OT license.
• Home health experience preferred.
• OASIS experience strongly preferred.
• Strong documentation and time management skills.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct in-home evaluations and functional assessments.

• Assist patients with ADLs (bathing, dressing, grooming, toileting).
• Train patients on adaptive equipment and home safety modifications.
• Complete OASIS documentation as required.
