Job summary

Caring Hand Home Health Solutions, Inc. is seeking an Occupational Therapist to provide in-home services on a per diem basis in Modesto, California.

You will help patients regain independence, improve function, and safely navigate daily life within their own homes. Applicants must hold an active California OT license, NBCOT registration, and have at least 2 years of clinical OT experience, CPR with AHA, and a valid driver’s license with reliable vehicle to travel to patient homes.

Qualifications

• Graduated from an occupational therapy curriculum accredited jointly by the AMA and AOTA committees.
• Registered with NBCOT.
• Active California Occupational Therapy license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.
• Valid driver’s license with reliable insured vehicle; ability to travel to patient homes.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in activities of daily living affected by illness or injury.
• Educate family members and caregivers on the patient’s treatment approach.
• Supervise OT Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Maintain accurate clinical records and report changes in patient condition.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services
• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
