Job summary

Caring Hand Home Health Solutions, Inc is seeking a per diem Occupational Therapist to help patients regain independence and navigate daily life in their own homes. You will develop and adjust treatment plans, educate families, and coordinate with physicians and the care team.

Requirements include graduation from an OT program, California OT license, NBCOT, current AHA CPR, and a valid driver's license with reliable vehicle.

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the relevant OT bodies.
• Registered to practice in California with a current OT license.
• Active CPR certification (AHA) and ability to travel to patient homes.

Responsibilities

• Evaluate patients in their home to identify needs and safety concerns.
• Develop personalized OT treatment plans and communicate with the care team.
• Teach daily living skills and train in adaptive equipment and devices.
• Maintain accurate clinical records and coordinate with the broader care team.
• Support discharge planning and supervise OT aides when applicable.

Please note that this role is per diem with a rate of $130 per visit.

Responsibilities

• Evaluate patients' functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
