Job summary

Caring Hand Home Health Solutions, Inc. seeks an experienced Occupational Therapist to provide in-home rehabilitation in California on a per diem basis. You will help patients regain independence and safely navigate daily activities with compassionate, patient-centered care.

Responsibilities include evaluating homes, creating treatment plans, teaching skills, coordinating with the care team, supervising OT assistants, and supporting discharge planning with travel to patient homes.

Qualifications

• OT degree from an accredited program.
• California license to practice OT (OTR).
• Current CPR certification (AHA).
• Registration for the OT licensure examination.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and promptly report changes in patient condition.
• Coordinate with the broader care team to ensure seamless, patient-centered care.
• Educate family members and caregivers on the patient’s treatment approach.
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
