Job summary

Caring Hand Home Health Solutions, Inc, based in California, seeks an Occupational Therapist for per diem visits to patients in their homes. You will help patients regain independence, improve function, and perform daily living activities safely, within a home health setting.

The role requires an OT degree, CA license, NBCOT registration, and a minimum of two years of clinical OT experience. CPR certification, a valid driver's license, and strong communication skills are essential.

Qualifications

• Graduate of an occupational therapy curriculum accredited by the American Medical Association and the American Occupational Therapy Association.
• Registered with the National Registration Examination of the American Occupational Therapy Association.
• Active California Occupational Therapy license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.
• Valid driver's license and reliable insured vehicle for travel to patient homes.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients' functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in activities of daily living affected by illness or injury.
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and promptly report changes in patient condition.
• Coordinate with the broader care team to ensure patient-centered care.
• Educate family members and caregivers on the patient’s treatment approach.
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.

• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver's license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
