Job summary

Caringhand Home Health Solutions, Inc is seeking an Occupational Therapist to help patients regain independence in their homes, delivered with compassionate, family‑oriented care.

The per diem role pays $130 per visit and requires a California OT license, NBCOT registration, CPR, and the ability to travel to patient homes. You will evaluate, plan, educate families, supervise staff, and assist with discharge planning.

Qualifications

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and AOTA.
• Registered with the National Registration Examination of the American Occupational Therapy Association (NBCOT).
• Active California Occupational Therapy license.
• Minimum 2 years of clinical OT experience.
• Current CPR certification with AHA.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns.
• Develop personalized treatment plans and communicate them to physicians and the care team.
• Teach and retrain patients in daily living skills affected by illness or injury.
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices.
• Maintain accurate clinical records and promptly report changes in patient condition.
• Coordinate with the broader care team to ensure seamless, patient-centered care.
• Educate family members and caregivers on the patient’s treatment approach.
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service.
• Support discharge planning as patients transition out of home health care.
• Perform other duties as required for patient services.
