Job summary

Caring Hand Home Health Solutions, Inc. seeks a per diem Occupational Therapist to support patients at home in California. You will assess home environments, craft individualized treatment plans, and help patients regain independence with daily living activities.

The role requires an active California OT license, NBCOT, and CPR certification, with travel to patient homes and supervision of support staff. Flexible, per diem schedule with potential for full-time conversion based on performance.

Qualifications

• Graduate of an OT curriculum accredited by ACOTE/CAOT.
• Active California Occupational Therapy license.
• NBCOT certification and registration.
• Minimum two years of clinical OT experience.
• Current CPR certification (AHA).
• Valid driver’s license and reliable insured vehicle.

Responsibilities

• Evaluate patients’ functional abilities and home environments for safety.
• Develop personalized OT treatment plans and communicate with physicians.
• Teach and retrain patients in activities of daily living.
• Recommend adaptive equipment and prosthetic/orthotic devices as needed.
• Maintain accurate clinical records and report changes promptly.
• Coordinate with care team for seamless patient-centered care.
• Educate families and caregivers on treatment approaches.
• Supervise OT Assistants and oversee home health aide care when OT is sole skilled service.
• Support discharge planning as patients transition from home health care.
• Perform additional duties to support patient services.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
