Job summary

Caringhand Home Health Solutions, Inc. is seeking an Occupational Therapist to provide in-home therapy in Placerville, California on a per-diem basis.

You will evaluate patients, develop personalized treatment plans, and coach families while coordinating with the care team. This role requires an active California OT license, NBCOT registration, CPR certification, and a valid driver's license with reliable vehicle.

Qualifications

• Graduate of an occupational therapy curriculum accredited by the AMA/ACOTE and AOTA
• Registered with the NBCOT
• Active California OT license
• Minimum 2 years of clinical OT experience
• Current CPR certification (AHA)
• Valid driver’s license and reliable insured vehicle; willingness to travel to patient homes
• Strong communication and organizational skills

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
