Job summary

Caring Hand Home Health Solutions, Inc. is seeking a licensed Occupational Therapist to help patients regain independence in their own homes. You will assess home environments, develop personalized treatment plans, and coordinate with physicians and the care team to ensure safe, effective care.

This per diem position offers $130 per visit, with a flexible schedule and opportunities for full-time conversion based on performance. Valid CA OT license and driver’s license required.

Qualifications

• Graduate of an occupational therapy curriculum accredited by major US associations.
• Registered with the National Board for Certification in Occupational Therapy or equivalent.

Responsibilities

• Evaluate patients’ functional abilities and home environments to identify needs and safety concerns
• Develop personalized treatment plans and communicate them to physicians and the care team
• Teach and retrain patients in daily living skills affected by illness or injury
• Recommend and instruct patients on adaptive equipment, splints, and prosthetic/orthotic devices
• Maintain accurate clinical records and promptly report changes in patient condition
• Coordinate with the broader care team to ensure seamless, patient-centered care
• Educate family members and caregivers on the patient’s treatment approach
• Supervise Occupational Therapy Assistants and oversee home health aide care when OT is the sole skilled service
• Support discharge planning as patients transition out of home health care
• Perform other duties as required for patient services

• Graduate of an occupational therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association
• Registered with the National Registration Examination of the American Occupational Therapy Association
• Active California Occupational Therapy license
• Minimum 2 years of clinical OT experience
• Current CPR certification with AHA
• Valid driver’s license, reliable insured vehicle, and the ability to travel to patient homes
• Strong communication and organizational skills
