Job summary

Diamond Care Group is seeking an experienced Occupational Therapist to provide in-home health services focused on improving patient independence and safety within their home. We hire the most thoughtful talent in the field and value dedicated professionals.

You will conduct in-home evaluations, develop and implement treatment plans, assist with ADLs, train on adaptive equipment, complete OASIS documentation, and collaborate with nurses and other team members.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Active Florida OT license.
• Home health experience preferred.
• OASIS experience strongly preferred.
• Strong documentation and time management skills.
• Valid driver’s license and reliable transportation.

Responsibilities
